The Brazos Valley Health Coalition (BVHC) was formed in January 2016 to act as a coordinated group to address health issues in the Brazos Valley. The BVHC, which consists of numerous social service, healthcare, education, and governmental agencies, has two main goals:
- Conduct a community health assessment (CHA) to identify the health needs and strengths of the Brazos Valley
- Create a community health improvement plan (CHIP) to determine health priorities and specific strategies that can be implemented to address the issues identified in the Community Health Assessment

Community Assessments
St. Joseph Health is currently performing a Community Health Needs Assessment to identify the most pressing needs of the communities we serve. This assessment helps guide future health programs, services, and policies to improve community well-being. It involves collecting data on health outcomes, socioeconomic conditions, and community input to ensure that the strategies align with actual community needs.
As a part of this work, we are conducting Community Partner Listening Sessions. Participation is needed from key stakeholders among a broad range of backgrounds, including but not limited to: community leaders, healthcare providers, non-profit organizations, faith-based groups, education professionals, public health agencies, resident leaders with lived experience in health challenges.
The time commitment includes a pre-survey that should take approximately 20 minutes or less to complete and a one-hour Zoom meeting.
During the session, participants will be asked to share their thoughts on health needs and issues within their community, discuss barriers to accessing care, social determinants of health (i.e. housing, transportations, food insecurity, income, etc), and opportunities for improving health, and offer suggestions for programs, services, or partnerships that could address the identified health needs.
A trained facilitator will guide the conversation to ensure that all voices are heard, and your contributions will be captured and analyzed as a part of the CHNA process.
